Results: Key result: 1.5 mile run performance in TiMs remains significantly faster than in women even after three years of testosterone reduction. It starts 21% faster, and then TiM's do slow down, but remain 12% faster than women in the third year. So testosterone reduction doesn’t remove male advantage

Key conclusion: the muscular advantage enjoyed by transgender women is only minimally reduced when testosterone is suppressed.

Just simply things like larger lunges, larger heart and greater blood volume gives athletes a sizable advantage. Hence why even auto-blood-transfusions (when a person 'donates' blood to be later used for their own treatment) are considered a performance enhancing treatment- and depending on the time elapsed between the transfusion and event is illegal in many sports.
