I think it's as simple as people understanding intuitively that sadomasochism is wrong and cruel to oneself and others, therefore they naturally associate it with something else that was deeply wrong and cruel. Everything stemming from sadism (or everything to do with masochism as well) can be linked together, and I believe this is why fetishes are famously comorbid psychologically.

Some no doubt are. One of the most notorious Nazi propagandists, Julius Streicher, was a well-known sadist who was almost always seen with a bullwhip, and the newspaper he published, Der Stürmer, often featured pornographic anti-Semitic material.

Gross. I can hardly stand anyone kink-identified any more. It's starting to piss me off more than trans, maybe because it seems more widespread and normalized. Although not as much a "civil rights" issue, maybe because kinksters - in my experience - thrive on being a subculture and people being freaked out by them.
