There's probably no better way to bring attention to the cruelty, barbarity, horror, misogyny and medieval nature of today's "gender affirming care" for TIFs and NBFs than to draw a parallel between the so-called "top surgeries" that girls and young women are being subjected to nowadays and the grisly torture, ghastly mutilation and savage sexual abuse that Saint Agatha was put through by woman-hating men when she was a teenage girl in Sicily 2,000 years ago.

This was very likely what she meant. The tweet is specifically about the game Dragon Age including mastectomy scars in their character creator. So she's doing a gotcha by saying "uh, dragons didn't exist either so what ??? you only care about top surgery scars because you're transphobic !"

It's a pretty common argument in medieval fantasy circles. I don't think it works, but I can't quite articulate why at the moment. Something about dragons not breaking the immersion because they actually don't exist at all, VS adding stuff that's clearly representative of modern day existence. So adding dragons make it medieval setting + magic stuff. No interference between the two. Adding elective mastectomy scars = modern religion and modern cosmetic surgery messing with my medieval roleplay.

It's a pretty common argument in medieval fantasy circles. I don't think it works, but I can't quite articulate why at the moment

In most magic heavy settings that are more medieval, the science based medical technology sucks because everyone relies on the church or mages for healing or whatever. But healing magic is usually depicted as being capable enough to not leave scars on skin (with the exception of mechanics like dark magic preventing healing or having the majority of healing magic too low level for that)

And if there isn't a lot of healing magic or it's very limited, you're just left with what was originally there in that time period. Apparently the concept has been around for a long time- apparently the ancient Egyptians were aware of breast tumors and tried mastectomies, but the ball wasn't really rolling until the 1800s because of how dangerous surgery used to be and the lack of anesthesia

If you had your breasts removed in the 1500s and managed to survive, I doubt the result would look anywhere nearly as clean as how that character creator has it

But also yeah, IMO dragons just make more sense because like…. there’s medieval art etc with dragons, they were clearly a thing back then, meanwhile things like top surgery/gender etc are so modern that it makes no sense
